Part One:
Please use attachment from program planning to write the program evaluation parts.
Executive Summary (Abstract) 1 page DUE OCT 5
Provide a brief overview of your report. Highlight the what and why and any major findings that resulted from the evaluation. Also include major conclusions and recommendations. The Executive Summary is often read by someone who does not have time to read the entire evaluation report, such as a representative from an external funding agency or another stakeholder.
Evaluation Questions and Program Objectives ( page-1 page)DUE OCT 5
Evaluation questions are basically research questions, and they directly connect to the program goals and objectives. To help you determine what you need to evaluate, identify the program goals and objectives. If you do not plan to evaluate the intervention created in HS 3073, you need to identify the goals and objectives for another program you have selected. Once you have identified the program objectives, then you can formulate your evaluation questions.
Part Two:
You have conducted a needs assessment, created a health promotion program based on your needs assessment data, and now you are planning the evaluation. In this class, you have created your evaluation questions and objectives based on the health promotion program. Each group will create the “mock” data used for the project. For this discussion board posting, please share how you feel about the evaluation process so far by responding to these questions: How do concepts of the evaluation process relate to other core concepts/responsibilities learned in the Health Studies program? Is the process what you thought it would be? Do you think this is the easiest or most difficult part of the health education/promotion process? Why?
