Use program Review of your selected agency.  write-up should be in the form of a narrative description of the agency.
1. Provide an inventory of programs/services; identify target population(s); and name of evidence-based interventions (EBIs) used by the agency (if any).
2. What are the agency policies that define the(se) program(s)? Formal structures? (For example: policies & procedures, rules & regulations, eligibility requirements, funding requirements, etc.)
3. What types of program and practice evaluations are currently being conducted at the agency? [HINT: See Chapter 4]
4. Who in the agency is involved in evaluations (if anyone)? What are their specific titles/positions?
5. List key funding/regulatory agencies, professional associations, and any other organizations that the agency is accountable to for their program(s).
